How to Apply
Getting started with Threshold Residential Services is easier than you might think. Just follow these simple steps, and our team will support you every step of the way.
Step 1: Reach Out to Your County Board of Developmental Disabilities
Begin by contacting your local County Board of Developmental Disabilities (e.g., Columbiana County). You’ll be assigned a Service & Support Administrator (SSA)—someone who works with you (or your loved one), your family, or guardian. Your SSA will help plan, coordinate, and monitor your personalized services.
Step 2: See if You’re Eligible
Your SSA will guide you through checking eligibility for Medicaid and Medicaid Waivers. If assistance is needed, they’ll help get you in touch with the Department of Job and Family Services to complete the application. If Medicaid doesn’t apply, don’t worry—there are other payment options, and we can walk you through those too.
Step 3: Figure Out Which Services You Need
Together with your SSA, you’ll go through an assessment to create an Individual Service Plan (ISP). This plan outlines the support and services you need to live your fullest life.
Step 4: Choose Your Service Provider
Once your Level of Care has been assessed, you can choose who will provide those services. If you’d like Threshold Residential Services to be your provider, just let your SSA know—and we’ll take it from there.
