Providing opportunities to our community.

We currently provide Routine and On-Site On-Call services to individuals in 22 residential locations. Services provided in these locations include 24/7 staffing with the supervision of all Activities of Daily Living, remote monitoring services, and respite care. We own and operate nine Licensed Residential Facilities, each of which are four-bedroom homes complete with cable, internet, and phone service. We also provide services in homes owned by the Columbiana County Board of Developmental Disabilities, homes owned by individuals served, family homes, and/or apartments where individuals reside.

Technology

We utilize Remote Support technology in 6 of our residential locations to support the independence of individuals served while keeping a person’s health and safety at the forefront of the Service Plan. Remote Supports can be used in place of or in conjunction with onsite staff and uses integrated whole-home technology to make an individual’s home a safe space. Two-way face-to-face tablets, door sensors, security cameras, fall sensors, automatic medication reminders, and kitchen appliance emergency shut-offs are just some examples of how we can support our individuals with cutting-edge Remote Support technology.

Transportation

We provide Homemaker Personal Care (HPC) transportation to individuals served in a residential setting or for community integrated activities. HPC transportation can be provided for medical appointments, grocery shopping, visiting friends, or for any other activity an individual desires to engage in.

Respite Care

Some families occasionally need a break from caring for a loved one with a developmental disability or experience emergency situations. we provide respite care for individuals needing short-term or long-term placement. Whether it’s for a weekend or for a few months, we’ll provide quality care to your loved one.

Representative Payeeship

Navigating through the process of Social Security or SSI/SSA Benefits can be a difficult task. Let us help you! At Threshold, we provide payee services to those individuals who receive residential care from the organization. Our Resident Benefits Coordinator will communicate with the Social Security Administration, Job and Family Services, the County Board of DD, and with other Threshold employees to ensure your benefits are maintained.
